If you would like to become a patient of Texas Family Medicine, please complete the applicable new patient forms found below. After completing these forms, call our office at 469-888-6575 to schedule an appointment or click here to request an appointment.

Please arrive at your first appointment 15 minutes early and bring your Driver’s License and Insurance card. Please also bring all of your medications (both prescription and over the counter) and the names and phone numbers of your previous PCP and any other physicians you have seen in the past.

Once you become a patient, you can enroll in our Patient Portal, an easy to use and secure website to manage your healthcare needs. It includes online features such as e-communication with our staff, obtaining test results, and the ability to request appointments.

What To Expect

Being well-prepared for your appointment will ensure that the doctor has all necessary information to provide you the best possible care. It also will help relieve any unnecessary anxiety you may be feeling.  Also, please take some time to get to know Dr. Noyes. We look forward to your first visit.
